Ever since ~
Means a state has continued from a certain event up to now. The second clause shows the continuing situation, such as ずっと〜ている or 一度も〜ない. It also follows nouns like 卒業 or 入社. It is more formal than てから and refers to a fairly long period.
日本に 来て以来、毎日日記を書いている。
Ever since I came to Japan, I have kept a diary every day.
大学を 卒業して以来、田中さんには一度も会っていない。
I haven't seen Mr. Tanaka once since we graduated from university.
Not until ~ / unless ~ first
Means "B cannot happen unless A is done first." The second clause is negative, such as できない, 分からない, てはいけない or 無理だ. からでなければ has the same meaning and is a little more formal. It shows that A is a necessary condition for B.
家族と 相談してからでないと、お返事できません。
I can't give you an answer until I've talked it over with my family.
実際に 使ってみてからでないと、いいかどうか分からない。
You can't tell whether it's good until you've actually tried it.

しんさつ is written 診察 (medical examination). Be careful: 察 looks like 祭 and 際, and 珍 looks like 診. The sentence means "Before the examination, my name was called at the reception."
