Finish doing completely; do something by mistake (regret)

〜てしまう has two uses. One is completion: doing something completely, to the end (宿題を全部やってしまいました, I finished all my homework). It often appears with 全部 or もう. The other is regret: you did something you did not mean to do, or something unfortunate happened (財布を落としてしまいました, I lost my wallet). The context tells you which meaning is intended.

I finished reading this whole book yesterday.

I left my umbrella on the train.

I'll get my homework done before dinner.

A: Is there still some cake? B: I'm sorry. My little brother ate it all.

注意

In conversation, short forms are common: 〜てしまう becomes 〜ちゃう and 〜でしまう becomes 〜じゃう (食べちゃった, 飲んじゃいました). These are casual forms for friends and family; use 〜てしまいました in polite situations and in writing.

"This morning I left my homework at home." しまいました follows the て-form of a verb, so 忘れて is correct. Here it shows regret about forgetting the homework. The dictionary form, the ます-stem and the た-form cannot come before しまう.

A asks "Is there still some juice?" and B says "Sorry, I drank it all." This is the casual short form of 飲んでしまった. When the て-form ends in で, でしまう becomes じゃう: 飲んで → 飲んじゃった. ちゃう is used only when the て-form ends in て.

"I've already lost my new umbrella. I'm in trouble." Because of 困りました, the speaker regrets losing the umbrella, so なくして しまいました fits. The other choices are a request, a suggestion and permission, which make no sense here.
