〜てはいけません expresses prohibition: "you must not ~." It is common for rules, manners and dangerous things, and is typically said by teachers to students or by parents to children. It can answer a 〜てもいいですか question (いいえ、〜てはいけません), but it sounds strong. When you ask a person directly not to do something, 〜ないでください is softer.

A: May I swim in this river? B: No, you must not. It's dangerous.
Because it is strong, do not use it to people above you or to customers. In speech, 〜てはだめです is also used, and ては / では often shorten to ちゃ / じゃ (入っちゃいけません, 泳いじゃいけません).

"This is a hospital. You must not make phone calls here." The て-form of かけます is かけて, so the answer is かけては. The other verb forms cannot be followed by は いけません.
"Everyone, you must not eat in the classroom. Please eat outside." The second sentence says to eat outside, so eating in the classroom is forbidden: 〜てもいいです gives permission, and 〜ましょう and 〜ませんか are invitations, so they do not match the second sentence.
A asks "Teacher, may I use a dictionary in the test?" and B answers "No, you must not." Because B says いいえ, permission is refused, which is expressed with 〜てはいけません. The other three choices all allow or tell A to use the dictionary, so they contradict いいえ.
